For most of the twentieth century, coffee sat on the list of things a cardiologist would tell you to cut back on. That advice has quietly reversed. The large prospective cohorts assembled over the last twenty years — hundreds of thousands of people, followed for decades — point consistently in one direction: moderate coffee drinkers tend to live longer and have fewer cardiovascular events than people who don’t drink coffee at all.
That is a real finding, and it deserves to be stated plainly. It also deserves the caveats that usually get stripped out of headlines. Let’s do both.
What the evidence actually says
The most useful way to read this literature is as a dose-response curve rather than a yes/no verdict. Across cohorts, risk of death from any cause falls as coffee intake rises from zero, reaches its lowest point somewhere around two to four cups a day, and then flattens out. It does not keep improving indefinitely.
A 2025 analysis of nearly 46,000 US adults from the National Health and Nutrition Examination Survey found that one cup a day was associated with a 16% lower risk of all-cause mortality, and two to three cups with 17%. Beyond three cups, no additional benefit appeared. Harvard’s long-running cohort work has reported a similar shape, with benefits observed up to about five cups daily.
The Harvard T.H. Chan School of Public Health summary is a fair characterisation of the field: moderate consumption is associated with lower risk of cardiovascular disease, and there is no good evidence that ordinary coffee drinking harms the healthy heart.
The three questions people actually ask
Does coffee raise blood pressure?
Acutely, yes — a cup of coffee produces a modest, temporary bump in blood pressure, typically a few mmHg, lasting a couple of hours. Chronically, the picture is different: habitual drinkers develop substantial tolerance to that pressor effect, and long-term cohort data do not show regular coffee consumption driving the development of hypertension. Does coffee cause dangerous heart rhythms?
This is one of the more durable myths. Randomised and observational work has largely failed to find that ordinary coffee intake increases clinically significant arrhythmia in healthy people. Palpitations — that flip-flop sensation — are common and usually benign. That said, individuals with a diagnosed arrhythmia should follow their cardiologist’s guidance rather than a population average.
Does coffee raise cholesterol?
Here the answer is genuinely “it depends,” and the variable is your brewing method. Coffee contains diterpenes — cafestol and kahweol — that raise LDL cholesterol. A paper filter traps almost all of them. A French press or a percolator does not. - Two to four cups a day is the range where the observed benefit is most consistent.
- Filter it. Paper-filtered brewing removes the LDL-raising diterpenes.
- Watch what goes in it. The mortality benefit in the Tufts data held for black coffee and coffee with low added sugar and saturated fat — not for sweetened, cream-heavy drinks.
- Mind the clock. Caffeine’s half-life is long enough that an afternoon cup can measurably degrade sleep.
The part the headlines leave out
Nearly all of this evidence is observational. Researchers cannot randomise fifty thousand people to drink coffee for twenty years, so they observe who drinks what and track outcomes. That design cannot fully separate coffee from the person drinking it. Coffee drinkers differ from non-drinkers in income, smoking history, physical activity, and dozens of other variables. Good studies adjust for those. No study adjusts for all of them.
There is also reverse causation to consider: people who feel unwell often cut back on coffee, which can make non-drinkers look sicker than they otherwise would. Careful cohorts try to handle this by excluding early events, but the concern never disappears entirely.
The honest summary: the evidence is strong enough to stop worrying about moderate coffee, and not strong enough to prescribe it as a treatment.
Where cacao enters the picture
Coffee’s cardiovascular story is largely about polyphenols, chlorogenic acids, and the absence of harm. Does adding milk or sugar cancel the benefit?
In the 2025 Tufts analysis, the mortality association held for black coffee and for coffee with low levels of added sugar and saturated fat. Heavily sweetened, cream-based coffee drinks did not show the same relationship.
Is decaf coffee just as good for the heart?
Decaffeinated coffee retains most of coffee’s polyphenols and has been associated with some of the same cardiovascular benefits in cohort studies, though the data set is smaller than for caffeinated coffee.
